1.) Current ID, Drivers license, or Passport 

2.) Most recent pay stub for W2 employee 

A. If you are “Self Employed” then please provide us your most recent bank statement

  B. If you just started your job then please provide a job letter from your current employer 

3.) Most recent “proof of residence” such as Phone bill, Light bill, or lease agreement
